A Case of Captopril-Induced Pancytopenia in Patient Undergoing CAPD

Abstract
Captopril, an angiotensin converting enzyme inhibitors, has been used increasingly for the treatment of hypertension and glomerulonephritis with proteinuria. Not only are they efficatious but the incidence of serious adverse effect with ACE
inhibitors
is similar to that with placebo. But they have been rarely associated with serious adverse effect such as agranulocytosis or pancytopenia.
A 58-year-old man undergoing peritoneal dialysis for end stage renal disease secondary to chronic essential hypertension was admitted because of sore throat and abdominal pain. The patient has taken captopril, nifedipine and furosemide for
fifteen
months. Seven days before he was admitted, sore throat, skin rash on the right lower leg were noted. On admission, CBC revealed pancytopenia. The captopril was discontinued and all other medications remained same. Pancytopenia and skin rash
improved
completely in eight hospital days.
We report a case of severe captopril-induced pancytopenia with a review of literature.
